Muddy Waters Restaurant-              6 High Priority Violations 5-Intermediate Violations 19 Basic Violations

Because of the number of Basic Violations we will only list the Intermediate and High Priority violations.

Intermediate Violation #1-Accumulation of black/green mold like substance inside the ice bin.At both sides of the bar.

Intermediate Violation#2-Accumulation of encrusted food debris on/around mixer head.

Intermediate Violation#3-Encrusted material on can opener blade.

Intermediate Violation#4-Ready to eat,potentially hazardous food prepared onsite and held more than 24 hours not properly date marked.Clam Chowder.

Intermediate Violation#5-Soda guns soiled on both sides of bar.

High Priority Violation#1-Wiping cloth sanitizing bucket stored near/on/above food preparation surface or food.

High Priority Violation#2-Single use gloves not changed as needed after changing tasks or when  soiled.Cook touched garbage by dish machine then continued to prep food with no glove change.

High Priority Violation#3-Nonexempt fish offered rawor undercooked has not undergone proper parasite destruction. Fish must be fully cooked or discarded

High Priority Violation#4-Live flies in kitchen.

High Priority Violation#5-Employee failed to wash hands before putting on on new set of gloves to  work with food.Cook working with raw chicken, failed to wash hands before putting on on new set of gloves

High priority Violation #6-Dishwasher handled soiled dishes or utensils and then handled clean dishes or utensils without  washing hands.corrected on site.

 

 

 

Pickle Barrel-3 High Priority Violations 1-Intermediate Violation 6-Basic Violations

Intermediate Violation#1-Employee with artificial nails/nail polish handling wax papers to wrap sandwiches without wearing gloves.

High Priority Violation #1-Toxic substance/chemical stored by or with food on bottom shelf of storage rack.Can of WD40 stored above prep area.

High Priority violation#-2-Potentially hazardous ,commercially in the process of reheating for holding more than 2 hours has not reached 135 degrees.Observed steam table not plugged in.

High Priority Violation#3-Employee failed to wash hands before putting on gloves to work with food.

 

 

 

Bruscos Italian Restaurant and Pizza- 1 High Priority Violation 1 Intermediate violation 5-Basic violations

Intermediate Violation#1-Ambient air thermometer in holding unit not accurate within plus or minus3 degrees,at walkin cooler

High Priority Violation#1-Raw animal food stored over ready to eat food.raw fish over veggies and raw chicken over cheese  walkin cooler.Warning

 

 

 

 

 

 

Nicks Pizza-6-High Priority violations 3-Intermediate Violations 8 Basic Violations

Intermediate Violation#1-Encrusted  material on can opener blade.

Intermediate Violation #2-No written procedures available for use of time as public health control to hold potentially hazardous food.Only one page of application observed.No complete procedure for pizza by the slice.

Intermediate Violation#3-Spray bottle wit toxic substance not labeled.Glass cleaner not labeled.

High Priority Violation-#1-Vacuum breaker missing at hose bibb in slop sink.

High Priority Violation#2-Toxic substance/chemical stored by or with single service items. Spray cleaner on shelf with deli paper.

High Priority Violation #3-Pizza by the slice no time markings Warning

High Priority Violation#4-Butter/margarine/garlic in oil-not held at proper temperature .Warning corrective action taken

High Priority Violation#5-Employee wiped hands on a dirty rag and continued food preparation without washing hands.Warning

High Priority Violation#6-Employee failed to wash hands before putting on gloves to work with food.Warning.

In our three part series of Dirty in Deerfield,there was only one restaurant with no violations at the time of inspection.Very important to remember this is what inspector finds  on any given day they enter a restaurant to be inspected it is a snapshot in time.
