Violations:
A summary of the violations found during the inspection are listed below. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division’s procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

36-33-4 Basic – Ceiling tile in disrepair. Water damaged with mold like build up near walkin freezer.
35A-03-4 Basic – Dead roaches on premises. 3 dead roaches in sushi bar area, 2 dead roaches in dishwashing area, 3 dead roaches at cookline, 1 dead roach inside soap dispenser in rear prep area and 6 dead roaches by time clock. On 4/1/15 Observed 10 dead roaches in sushi bar area and 13 dead roaches in dishwashing area. On 4/2/15 Observed 5 dead roaches in sushi bar area, 2 dead roaches at cookline, 1 dead roach by drink station, 2 dead roaches by in dishwashing area and 1 dead roach in rear prep area.
14-70-4 Basic – Ice buildup in walk-in freezer.
29-11-4 Basic – Leaking pipe at plumbing fixture. At prep sink and at handwash sink in rear prep area.
35A-05-4 High Priority – Roach activity present as evidenced by live roaches found. Observed approximately 51 live roaches in sushi bar area, 69 live roaches by dish machine, 1 live roach on top of prep table at cookline while food preparation was taking place, 2 live roaches under prep table at cookline, 1 live roach under handwash sink at cookline, 2 live roaches under table by grill in rear prep area, 16 live roaches by dry storage shelving near kitchen entry way, 1 live by time clock and 1 live on rolling cart near sushi bar area. On 4/1/15 observed 5 live roaches by time clock in dry storage area by cookline, 6 live roaches under prep table at cookline, 4 live roaches under cart by drink station, 1 live roach by drink station, 2 live roaches in gaskets at Daewoo cooler #1in sushi prep area, 1 live roach under rice pot storage table in sushi bar area, 3 live roaches in gaskets at Daewoo cooler #2 in sushi prep area , 1 live roach near handwash sink in rear prep area, and 3 live roaches under prep table by grill in rear prep area. On 4/2/15 observed 12 live roaches in sushi bar area , 7 live in dishwashing area, 5 live roaches in cookline area, 1 live roach in drink station, 1 live roach in dry storage shelving
